Payment Agreement *

By checking this box, you agree to the following payment terms: A nonrefundable $50 deposit is required to confirm your booking. This amount will be applied to your final balance. Full payment is due before the makeup application begins. If I am traveling to your location, full payment must be submitted at least 3 hours prior to my scheduled departure time. In the unlikely event that you are not satisfied with your makeup look, a full refund will be issued (excluding the travel fee, if applicable). In this case, the makeup will be removed before I leave.
